Locum Social Worker - Adults Integrated Discharge Team - Whiston, UK

Salary: £35 Hourly

Type: Ongoing Full Time Hours: 37 hours per week , between 8.45am and 5.15pm with a mixture of working in the office and working from home on a rota basis.

What you will do:

- Assess patients' needs and develop comprehensive plans to facilitate their smooth discharge from the hospital.
