What you will do:

* Serve as the primary contact point for referrals into Adult Social Care

* Screen and assess referrals, ensuring appropriate and timely responses

* Signpost to relevant services and provide Information, Advice, and Assistance

* Work collaboratively with the Early Response team to coordinate service delivery

* Maintain accurate records and documentation in line with organisational requirements

* Engage with a range of stakeholders, building effective relationships to enhance service provision
