What you will do:

Undertake statutory reviews of care and support plans in accordance with the Care Act 2014, ensuring services remain appropriate and outcome-focused.
Complete social care assessments for adults and carers, including reassessment of needs where circumstances have changed.
Carry out Mental Capacity Assessments and contribute to Best Interests decisions in line with the Mental Capacity Act 2005.
Identify and respond to safeguarding concerns, progressing issues appropriately in line with local procedures.
Develop and review care and support plans, ensuring a strengths-based and person-centred approach.

Person Specification:

Qualifications

Hold a degree in Social Work or an equivalent recognised qualification.
Registered with Social Work England.

Registration & Compliance

Possess knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks.
Understand CQC standards and local authority statutory duties.
Enhanced DBS clearance is required, ensuring you meet the highest standards of professional practice.
