We have an exciting opportunity for an experienced Adult Social Worker to join the Transition Team in Hereford. This full-time locum assignment offers a competitive hourly rate of £31.97, working 37 hours per week over an initial 12-week contract, commencing 3rd August 2026.
Based at Plough Lane, Hereford, you will play a vital role in supporting young adults aged 18-25 years as they transition from Children's Services into Adult Social Care. Working within a supportive multidisciplinary team, you will deliver strengths-based, person-centred interventions that promote independence, wellbeing, and positive long-term outcomes.

What you will do:
- Complete comprehensive, strengths-based assessments in accordance with the Care Act 2014.
- Identify eligible care and support needs, developing person-centred care plans that promote independence and maximise wellbeing.
- Support young people aged 18-25 years as they transition from Children's Services into Adult Social Care.
- Undertake Section 42 Safeguarding Enquiries, ensuring the safety and wellbeing of vulnerable adults.
- Complete Mental Capacity Act (MCA) Assessments and facilitate Best Interests decision-making processes where appropriate.
- Contribute to Continuing Healthcare (CHC) assessments and multidisciplinary decision-making.
- Participate in court work and prepare professional reports when required.
- Work collaboratively with young adults, families, carers, education providers, healthcare professionals, and partner agencies to ensure effective transition planning.
- Maintain accurate, timely, and comprehensive case records in line with organisational policies, statutory guidance, and Social Work England standards.
- Promote strengths-based practice, safeguarding principles, and positive risk management throughout all aspects of service delivery.
Requirements:
- Degree or equivalent qualification in Social Work.
- Current registration with Social Work England.
- Experience working within Adult Social Care, particularly with Transition Services, Learning Disabilities, Physical Disabilities, or Mental Health.
- Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, Care and Support Statutory Guidance, Continuing Healthcare, and safeguarding legislation.
- Excellent assessment, communication, analytical, and multidisciplinary working skills.
- Ability to manage a varied caseload and work effectively within statutory timescales.
