What you will do
• Assessment and Support Planning: Undertake strengths-based and needs-led assessments, developing personalised care and support plans.
• Hospital Admission Prevention: Support adults and carers through interventions designed to reduce unnecessary hospital admissions and promote community-based care.
• Safeguarding: Conduct safeguarding enquiries in accordance with the Care Act 2014, ensuring individuals receive appropriate protection and support.
• Multi-Agency Working: Collaborate with health professionals, community teams, carers, and partner organisations to deliver coordinated services.
• Communication: Effectively communicate complex information to individuals, carers, and professionals using a range of methods and approaches.
