What you will do
- Case Management: Manage a varied caseload of adults with complex physical, mental health, learning disability, and age-related support needs.
- Assessment: Complete strengths-based assessments in accordance with the Care Act 2014.
- Care Planning: Develop, implement, and review person-centred care and support plans.
- Mental Capacity: Undertake Mental Capacity Act assessments and facilitate Best Interest decision-making where appropriate.
- Safeguarding: Carry out Section 42 safeguarding enquiries to protect vulnerable adults from harm.
- Multi-Agency Working: Collaborate with health professionals, community organisations, carers, and partner agencies to achieve positive outcomes.
- Promoting Independence: Support adults to maximise independence, wellbeing, choice, and social inclusion.
- Record Keeping: Maintain accurate case records and prepare high-quality assessments, reports, and documentation.
- Professional Practice: Participate in multidisciplinary meetings, supervision, and continuous professional development activities.
