<li>To provide a high-quality and effective professional casework service, taking responsibility for working and managing a caseload within a framework of appropriate supervision, dependent upon the complexities of these cases and the social worker’s experience.</li><li>Manage a caseload within a framework of appropriate reflective supervision, undertaking focused, analytical assessments, SMART plans and reviews that have clear objectives and outcome measures in order to ensure that all vulnerable children receive a service that reflects the Council’s commitment to safeguarding and putting the needs of individual children first.</li><li>Demonstrate and model good practice that reflects a commitment to relationship and strengths-based practice, working collaboratively with children, young people and their families.</li><li>Work directly with children and young people to establish their wishes and feelings, using a variety of techniques, as required.</li><li>Use the Signs of Safety practice model in assessments and interventions with children and their families.</li><li>Maintain accurate and up to date records in line with Data Protection legislation (GDPR) and use Medway Council specific recording systems to promote effective case management.</li><li>Prepare clear, structured and concise reports for meetings, i.e. Child Protection Conferences, strategy meetings, Looked After Children Reviews, Children in Need meetings, and court proceedings as necessary, to the required standard and within the prescribed timescales.</li><li>Keep up to date with research findings, theoretical models and innovative practice within the social work field in order to maintain the highest professional standards.</li><li>Participate in regular supervision with your manager to review casework and critically reflect on, and identify any areas of learning.</li><li>Keep up to date with the Council’s organisational policies, government guidance, legislation, research and national trends in order to maintain the highest professional standards.</li></ul>

Main duties:<ul>
<li>To take responsibility for a suitable caseload and develop experience and competency across the following duties with opportunity to progress from one grade to the next when/if progression requirements are met.</li><li>Develop and maintain effective relationships with service users, families, carers and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to service planning that is person-centred and strengths-based.</li><li>Construct and implement care packages which are clearly linked to the needs of clients, working creatively with community and voluntary organisations to maximise independence, to minimise risks and improve outcomes and the quality of life for service users and their carers.</li><li>Be responsible for the assessment and safe discharge of service users from hospital back into community settings, working within challenging timeframes and reducing risk of future re-admission.</li><li>Undertake Mental Capacity assessments for service users where it is deemed that they may lack the mental capacity to make a specific decision.</li><li>Identify service users who are eligible for Continuing Health Care (CHC) or Section 117 funding and undertake checklists and work with other professionals to provide a multi-disciplinary assessment within this framework.</li><li>Take responsibility for managing an individual caseload of clients, regularly monitor, review and evaluate changes in clients needs and ensure that all services contribute effectively to the support/care plan, and it remains an effective use of resources.</li><li>Intervene in emergency situations to protect vulnerable adults and children and initiate the appropriate statutory or other actions required.</li><li>Maintain and update case notes and other records within required timescales, write reports provided written responses as required, if required, give evidence in court in relation to care or other proceedings.</li><li>Develop and maintain up to date relevant knowledge and competency and personal effectiveness in relevant legislation, social work practice and changes in social work including strengths-based working delivery models through development as agreed by the Team Manager.</li><li>Fully participate in individual supervision and performance appraisal as well as participate in the development and implementation of performance indicators and quality service standards, always striving for improved performance both personally and as a service.</li></ul>
