This role begins with a 12-week period with the possibility of extension, giving you the perfect chance to dive into meaningful work with our team. You'll have the chance to work Monday through Thursday from 8:45 to 17:00, and on Fridays from 8:45 to 16:00, allowing you to balance professional life with some well-deserved downtime.

What you will do:

- Effectively manage a complex and diverse caseload of young people, including supporting care leavers and former and current asylum-seeking children.
- Conduct thorough needs assessments and develop plans tailored to each care leaver's individual needs.
- Produce detailed and outcome-focused pathway plans guiding the transition to independence and adulthood.
- Collaborate with a variety of agencies and professionals to promote positive outcomes for young people.
- Build enduring and trustworthy support networks for care leavers, ensuring their voice and opinions are central in their care planning.
- Encourage care leavers' participation in decision-making and service development.
- Work independently yet collaboratively, nurturing strong relationships with both care leavers and their families.
- Maintain high standards of documentation by producing clear and concise reports.
- Actively participate in your professional development, embracing training opportunities to enhance your role.
