What you will do

Manage a caseload of adults with varied health and social care needs, ensuring personalised support.
Complete strengths-based assessments, care planning, reviews, and risk assessments to tailor care plans.
Collaborate effectively within a multidisciplinary team of health professionals and partner agencies.
Develop and implement person-centred care and support plans promoting independence and wellbeing.
Support carers by identifying their needs and ensuring they receive the necessary help and resources.
Apply relevant social care legislation and statutory duties in all aspects of your practice.
Maintain accurate and up-to-date case records in line with council policies and professional standards.
