<li>Identify service users who are eligible for Continuing Health Care (CHC) or Section 117 funding and undertake checklists and work with other professionals to provide a multi-disciplinary assessment within this framework.</li>

<li>Complete Human Rights Act assessments for those who have no recourse to public funds but may require support under the Care Act.</li>

<li>Take responsibility for managing an individual caseload of clients, regularly monitor, review and evaluate changes in clients needs and ensure that all services contribute effectively to the support/care plan, and it remains an effective use of resources.</li>

<li>Undertake ongoing and urgent case reviews as necessary.</li>
