<li>Undertake comprehensive, person-centred assessments of need, risk, and mental capacity, ensuring outcomes are tailored to individual circumstances.</li><li>Develop and review care plans that promote recovery, independence, and well-being, working in partnership with service users and their support networks.</li><li>Deliver statutory duties under the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.</li><li>Facilitate access to appropriate services, including community resources and specialist mental health interventions.</li><li>Lead on high-risk and complex cases, ensuring timely and effective interventions.</li><li>Act as the Investigating Officer in safeguarding adult cases, coordinating multi-agency responses and managing associated risks.</li><li>Provide expert advice and decision-making support to colleagues and partner agencies.</li><li>Prepare high-quality reports for legal proceedings, tribunals, and multi-agency case reviews.</li><li>Provide professional supervision and mentoring to less experienced social workers and other team members.</li><li>Support the induction of newly qualified social workers and students, acting as a Practice Educator where appropriate.</li><li>Promote reflective practice and professional development within the team, contributing to a learning culture</li><li>Work closely with health, housing, voluntary sector, and other professionals to deliver coordinated care.</li><li>Represent the Adult Mental Health Team at multi-agency meetings, contributing to joint decision-making and planning.</li><li>Build positive relationships with partner organisations to enhance service delivery and outcomes for service users.</li><li>Contribute to service development, identifying areas for improvement and implementing innovative practices.</li><li>Ensure accurate and timely record-keeping in line with organisational policies and professional standards.</li><li>Participate in audits, reviews, and inspections to maintain high-quality service provision.</li></ul>
&nbsp;Requirements of the role:<ul>
<li>A recognised Social Work qualification (e.g., BA/MA in Social Work, DipSW, or equivalent).</li><li>Registration with Social Work England.</li><li>Substantial post-qualification experience in adult mental health services, including complex case management.</li><li>In-depth knowledge of relevant legislation, including the Care Act 2014, Mental Health Act 1983, and Mental Capacity Act 2005.</li><li>Experience in safeguarding adults and managing risk in multi-agency contexts.</li><li>Advanced assessment, planning, and decision-making skills.</li><li>Strong communication and interpersonal skills, with the ability to engage effectively with service users, carers, and professionals.</li><li>Leadership and mentoring skills, with the ability to inspire and support others.</li><li>Resilience and adaptability in a demanding, fast-paced environment.</li></ul>
