What you will do:
- Provide tailored and evidence-based care to a caseload of psychosocial patients, ensuring the care is shaped by individual needs and feedback.
- Deliver a structured programme of psychosocial interventions, sharing essential information and advice on substance misuse, smoking cessation, blood-borne viruses, and safe sex practices.
- Engage with patients in a non-judgemental manner, both one-to-one and in group settings, supporting nursing and pharmacy teams in a variety of healthcare environments, including wings and house blocks.
- Follow our bespoke HiJ competency framework, adhering to our policies and procedures with a strong focus on safeguarding.
Requirements:
- Demonstrable evidence of continued professional development (CPD).
- A relevant degree or qualification, or equivalent experience in the field.
- Strong knowledge and experience in delivering structured psychosocial interventions.
- Proven experience working within a multi-disciplinary framework that encompasses housing, social care, healthcare, and mental health pathways.
