What you will do

Team Leadership: Lead and manage a team of Personal Advisors providing support to care leavers aged 16 to 25.
Supervision and Support: Deliver regular supervision, performance management, and professional support to team members.
Care Planning: Oversee the development, implementation, and review of Pathway Plans to ensure young people receive effective and person-centred support.
Quality Assurance: Monitor service delivery, ensuring high standards of practice and compliance with statutory responsibilities.
Service Improvement: Identify opportunities for service development and implement strategies to enhance outcomes for care leavers.
Partnership Working: Build and maintain effective relationships with local authorities, education providers, housing services, health professionals, and community organisations.
Performance Monitoring: Monitor team performance against key service targets and statutory requirements.
Safeguarding: Ensure safeguarding responsibilities are effectively managed and that young people's welfare remains central to all decision-making.
