Social Worker within the Ageing Well Team in Wolverhampton, earning £31.97 per hour. This exciting locum opportunity offers a 3-month full-time contract, working 37 hours per week from Monday to Friday, 9:00am to 5:00pm. Working with older adults, their families, carers, and partner agencies, you will play a vital role in promoting independence, wellbeing, and choice through strengths-based practice and person-centred interventions.

What you will do:

• Manage a caseload of older adults with a variety of complex needs and circumstances.
• Undertake strengths-based assessments in accordance with the Care Act 2014.
• Develop, implement, and review person-centred care and support plans that promote independence and wellbeing.
• Complete risk assessments and implement interventions to reduce risks while maximising independence.
• Lead safeguarding enquiries and interventions in line with relevant legislation and local procedures.
• Undertake Mental Capacity Assessments and support Best Interest decision-making processes where required.
• Facilitate effective hospital discharge planning and support individuals to access appropriate community services.
• Work collaboratively with NHS professionals, care providers, housing services, carers, and voluntary sector organisations.
• Undertake Carers Assessments and support carers to access appropriate resources and services.
• Maintain accurate, timely, and professional case records in line with statutory and organisational requirements.
• Contribute to service development, reflective practice, and continuous improvement initiatives.
