About the Role We're have an exciting opportunity for a Support Coach to join our new Independent Approved Premises (IAP) covering the day shift on a rolling rota. You will play a key role in supporting our residents who have left prison and are based within a residential setting to reintegrate into the community. You will develop and engage with our residents to produce tailored support plans which support their individual needs in successful rehabilitation and reintegration within the community. You will identify their needs, goals and skills, and then support and motivate them to achieve their potential. You will support them in engaging with activities, the community, and provide useful tools and resources for better life skills management. The service holds a strong focus on public protection and effective risk management. Key Responsibilities include:<ul><li>Ensuring the safety and wellbeing of residents and participants at all times</li><li>Engaging with residents, ensuring they are adhering to their license conditions. Main duties:

<ul>
<li>To be the first point of contact for Adult Social Care and other related services and to provide high quality and comprehensive support to empower users and carers to plan, work towards and achieve their outcomes.</li>

<li>To use judgement, assessment and decision-making skills to identify and allocate appropriate resources to meet customer needs and requirements, determine eligibility for services and other support and assistance, including the assessment of risk to service users. This includes practical assistance in terms of the provision of disability equipment and home adaptions to users with a physical disability in order to maintain their maximum independence within the community.</li>

<li>Working effectively individually and as part of a team, to achieve service outcomes and actively contribute to the development and smooth running of the service in accordance with local policies procedures and legislation.</li>

<li>To ensure that all related administrative functions are undertaken.</li>

<li>To maintain a clear understanding of the objectives, direction, and development of the service area to meet changing needs, with emphasis on customer relations, continuous service improvement and ensuring that a positive image of the Council's is portrayed at all times.</li>
