What you will do:
- Manage a complex caseload of adults with learning disabilities in the South Leeds area.
- Conduct Care Act assessments, reviews, and reassessments to pinpoint eligible needs and personal objectives.
- Create and update support plans that enhance independence, choice, and wellbeing.
- Execute safeguarding enquiries and manage concerns in accordance with statutory responsibilities.
- Assess and manage risks effectively, implementing safeguarding and risk management strategies.
- Work within the Mental Capacity Act framework to complete Mental Capacity Assessments and contribute to Best Interest decisions.
- Commission suitable care and support services aligned with assessed needs and personal goals.
- Collaborate with individuals, families, carers, and partner agencies to support adults in achieving meaningful goals.
- Make community visits for assessments and to support people in their varied locations across Leeds.
- Maintain meticulous case records in line with Leeds City Council procedures and Social Work England standards.
