What you will do:
In this exciting role, you will be responsible for conducting Care Act needs assessments, creating support plans, and processing commissioning of care applications. You will help Birmingham citizens transition smoothly from short-term to long-term care services post-hospital discharge.
Completing essential assessments such as S117 Mental Health, Continuing Health Care, and Mental Capacity Act assessments will be required. Ensuring the safeguarding of individuals is a top priority.
You will work in partnership with multi-professional teams, coordinating visits and assessments in the community and at Summer Hill Terrace to ensure comprehensive support.

Requirements of the Social Worker:
To be successful in this role, you must have a degree or equivalent in social work, and current Social Work England registration is essential. We expect significant frontline experience within UK Social Services, demonstrating your capability as a qualified social worker.
