What you will do:

• Work with children with disabilities and their families to assess needs and provide appropriate support.
• Develop, implement, and review personalised care plans focused on improving outcomes for children and young people.
• Collaborate with multidisciplinary teams and partner agencies to deliver coordinated support.
• Monitor and review cases to ensure children’s welfare and needs remain central to decision-making.
• Provide guidance, advice, and support to families, helping them access the services they need.
• Maintain accurate records, assessments, and reports in line with professional standards and statutory requirements.
