What you will do:
- Undertake Care Act 2014 compliant assessments, reviews, and support planning to identify eligible needs and outcomes
- Manage a complex caseload, ensuring timely interventions, accurate record-keeping, and outcome-focused practice
- Lead and contribute to adult safeguarding enquiries, ensuring risks are identified, managed, and mitigated
- Carry out Mental Capacity Act assessments and Best Interest decisions where required
- Work collaboratively within multi-disciplinary teams including NHS partners, housing services, and community organisations
Person Specification:
Qualifications:
- Degree or postgraduate qualification in Social Work
- Completion of ASYE (or willingness if newly qualified)
Registration and Compliance:
- Registered with Social Work England
- Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, Safeguarding Adults frameworks and Equality Act 2010
