What you will do
• Mental Health Assessments: Conduct comprehensive Mental Health assessments to identify needs, risks, and appropriate intervention pathways.
• Front Door Screening: Support the Contact Team by ensuring referrals are assessed promptly and effectively.
• Decision Making: Provide professional analysis and recommendations to ensure individuals receive appropriate support and services.
• Crisis Intervention: Respond to urgent situations and provide advice and intervention where required.
• Multi-Agency Working: Collaborate with healthcare professionals, community services, mental health providers, and partner agencies.
• Care Planning: Contribute to short-term intervention planning and recommendations for ongoing support where appropriate.
• Record Keeping: Maintain accurate, timely, and comprehensive case records in accordance with statutory and organisational requirements.
