What you will do:

- Conduct assessments and reviews of children with disabilities and their families
- Develop and implement care plans that address the unique needs of each child
- Support families in navigating services, addressing challenges, and accessing resources
- Work collaboratively with family support workers and direct payments workers
- Maintain accurate records and prepare reports for case management
- Participate in multi-agency meetings to ensure coordinated support for families
