This is an exciting opportunity for one full-time Social Worker to join our dynamic Home First Team within Swansea Council on a temporary basis for an initial period of 3 months. What you will do:

* Work collaboratively within the Home First Team and local hospitals to facilitate safe and timely discharges for patients.

* Conduct comprehensive assessments of individuals' needs in hospital and community settings.

* Develop and review care plans, ensuring they are tailored to meet the specific needs of each individual.

* Liaise with multidisciplinary teams, families, and external agencies to coordinate effective care and support.

* Support individuals in regaining independence and integrating back into their communities.
