  • Transitions Assessment & Planning: Complete transition assessments for young people approaching adulthood in line with the Care Act 2014, developing person-centred plans aligned with Preparing for Adulthood outcomes.

  • Multi-Agency Working: Work collaboratively with children's social workers, adult social care teams, SEND services, and other partners, attending reviews and professional meetings.

  • Disability & Additional Needs Support: Support young people with learning disabilities, autism, and complex needs, promoting independence and community inclusion.
