What you will do
• Case Management: Manage a varied caseload of young people transitioning from Children's Services to Adult Social Care.
• Assessment: Undertake strengths-based assessments in accordance with the Care Act 2014.
• Care Planning: Develop, implement, and review personalised care and support plans that reflect individual needs, aspirations, and outcomes.
• Transition Planning: Work collaboratively with Children's Services, Adult Social Care teams, and partner agencies to ensure smooth and effective transition arrangements.
• Family Support: Provide guidance and support to young people and their families regarding available services, resources, and funding options.
• Mental Capacity Assessments: Complete Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005.
• Safeguarding: Identify, manage, and respond appropriately to safeguarding concerns.
• Advocacy: Promote independence, inclusion, self-determination, and person-centred practice.
