What you will do

  • Assessment: Conduct holistic, strengths-based assessments to identify care and support needs.
  • Care Planning: Develop, implement, and review person-centred care plans.
  • Multi-Agency Working: Work closely with healthcare professionals and community services to ensure integrated care.
  • Advocacy: Promote patient-centred approaches, ensuring individuals' rights and preferences are respected.
  • Case Recording: Maintain accurate and timely records using systems such as Mosaic.

Qualifications / Requirements

  • Qualification: Degree in Social Work or equivalent.
  • Registration: Registered with Social Work England.
  • Experience: Recent experience within hospital teams or Transfer of Care settings.
  • Skills: Strong assessment, communication, and decision-making abilities.
  • IT Knowledge: Familiarity with Mosaic system is desirable.
