What you will do

• Work with a locality-based team to support and protect vulnerable individuals and families

• Complete comprehensive assessments and risk analyses

• Develop and review tailored care and protection plans

• Work in partnership with multi-disciplinary professionals and agencies

• Provide practical guidance and support to service users

• Maintain accurate records and prepare reports in line with statutory requirements
