What You Will Do:

• Conduct thorough assessments of individuals and families to identify any safeguarding issues.
• Develop and implement effective care plans to address identified needs.
• Collaborate with multi-agency teams to safeguard adults and children at risk.
• Provide guidance and support to families to help them navigate through challenges.
• Maintain accurate records and report writing for case management purposes.
• Attend and contribute to case conferences and review meetings.
