What you will do

Social Care Reviews: Conduct comprehensive social care reviews, ensuring services remain appropriate and effective.
Assessment: Complete Care Act assessments and contribute to the development of support plans that meet individual needs.
Case Management: Manage a caseload of approximately 15 to 18 adults, ensuring timely interventions and high-quality outcomes.
Partnership Working: Collaborate closely with the Floating Support Team to provide coordinated and effective support.
Care Planning: Promote independence and wellbeing through person-centred care planning and review processes.
Legislative Compliance: Work in accordance with the Care Act 2014, safeguarding procedures, and Adult Social Care legislation.
Record Keeping: Maintain accurate, timely, and compliant case records, assessments, and reports.
Outcome-Focused Practice: Support individuals to achieve positive outcomes while maximising independence and choice.
