What You Will Do

Carry out comprehensive community care and safeguarding assessments in line with statutory requirements, identifying risks, needs, strengths, and support options.

Develop, implement, and review person-centred care plans tailored to individual needs, ensuring services remain effective and appropriate.

Commission and coordinate cost-effective care packages that reduce risk and enhance quality of life for service users and carers.

Work collaboratively with individuals, families, carers, and community networks to support informed decision-making and promote independence.

Manage an allocated caseload effectively, undertaking regular reviews to ensure the best use of resources and high-quality service delivery.

Build and maintain strong relationships with clients and their support networks, offering guidance and helping navigate any conflicts or challenges.

Liaise with internal colleagues and external agencies to gather essential information and ensure coordinated, holistic care planning.

Maintain accurate and up-to-date case records, preparing reports and presenting evidence in court where required.

Supervise social work students on placement and support staff or volunteers, contributing to a learning-focused and collaborative team environment.

Engage in post-qualifying training and development, meeting requirements set by the Care Council for Wales and working towards experienced practitioner awards.
