What you will do:

  • Coordinate care for complex cases under the Care Programme Approach (CPA), ensuring tailored support for each service user.
  • Develop collaborative, person-centred care plans aligned with individual needs and goals.
  • Conduct comprehensive risk assessments and implement effective risk management strategies.
  • Safeguard vulnerable adults using legislation including the Mental Health Act, Care Act, and Mental Capacity Act.
  • Carry out holistic mental health and social care assessments, including Care Act assessments.
  • Work closely with carers, families, and wider support networks to deliver comprehensive care.
  • Build strong partnerships with statutory and voluntary agencies to enhance service delivery.
  • Liaise with inpatient units to ensure continuity of care across services.
  • Maintain accurate, up-to-date records and produce high-quality reports.
