About the Role

We are seeking an experienced Mental Health Social Worker to join the Adult's Mental Health Team in Stockport on an ongoing basis. This is an exciting opportunity offering full-time hours with a hybrid working model, allowing you to work from home 1-2 days per week when meetings and service needs permit.

You will play a vital role in supporting adults experiencing mental health challenges, delivering high-quality statutory services and working collaboratively within a multi-disciplinary environment to promote recovery, independence and wellbeing.

Key Responsibilities

Assessment & Care Planning

• Conduct comprehensive assessments for adults experiencing mental health difficulties

• Develop, implement and review person-centred care and support plans

• Undertake risk assessments and formulate effective risk management strategies

Crisis Intervention & Support

• Engage in crisis intervention where required

• Provide guidance and emotional support to service users and their families

• Promote recovery-focused and strengths-based practice

Multi-Agency Working

• Work collaboratively with health professionals, housing services and voluntary agencies

• Attend and contribute to multi-disciplinary meetings

• Communicate assessment outcomes and care plans effectively to stakeholders

Professional Practice & Compliance

• Maintain accurate, up-to-date case records in line with statutory and departmen
