About the Role

We are seeking a skilled and motivated Social Worker to join the Adult's Mental Health Team in Calderdale on an ongoing basis. As part of the Community Mental Health Team (CMHT), you will deliver high-quality statutory social work services, promote recovery-focused practice, and work collaboratively with multi-agency partners to ensure person-centred support.

Key Responsibilities

Statutory Duties & Assessments

• Undertake assessments and reviews under the Care Act 2014

• Develop strength-based, person-centred care and support plans

• Complete Mental Capacity Act assessments and facilitate Best Interests decisions

• Lead Section 42 safeguarding enquiries

• Conduct comprehensive risk assessments relating to mental health, self-neglect, exploitation and vulnerability

• Develop and implement risk management and contingency plans

Mental Health Act & Legal Frameworks

• Work in accordance with the Mental Health Act 1983 (as amended)

• Support or undertake Mental Health Act assessments where required

• Liaise with Approved Mental Health Professionals (AMHPs) and health colleagues

• Contribute to Tribunal reports and statutory reviews

Multi-Agency & Partnership Working

• Contribute to discharge planning and community transitions
