This full-time role sits at the heart of the Living Well Network Alliance, an innovative, integrated partnership delivering community-based mental health services alongside organisations including South London and Maudsley NHS Foundation Trust, Certitude, Thames Reach, and Lambeth NHS CCG.

• Complete strength-based assessments and develop person-centred support plans.

• Actively contribute to safeguarding adults, ensuring risks are identified, managed, and escalated appropriately.

• Maintain accurate, timely, and legally compliant case records in line with statutory guidance.

• Apply evidence-based practice, drawing on research, online journals, and professional resources.
