Social Worker - Mental Health Hospital Team

We have an exciting ongoing locum opportunity for an experienced Mental Health Social Worker to join a Mental Health Hospital Team in central London. This role is ideal for a practitioner passionate about supporting individuals in mental health placements and enabling successful transitions toward more independent living. As part of the Adult Social Care Mental Health Service, you will work collaboratively within the Living Well Network Alliance, an innovative partnership alongside South London and Maudsley NHS Foundation Trust, Certitude, Thames Reach, and Lambeth NHS Clinical Commissioning Group, delivering high-quality, coordinated mental health services to Lambeth residents.

What you will do:

  • Assessments: Complete comprehensive assessments for individuals within mental health placements.

  • Care Planning: Develop and implement personalised care and support plans to promote independence.

  • Multi-Disciplinary Working: Work closely with NHS colleagues, housing providers, voluntary organisations, and alliance partners.

  • Safeguarding: Safeguard and promote the welfare of vulnerable adults, ensuring statutory duties and procedures are met.

Salary range
