What you will do:
- Provide a high-quality social work service to adults and their families, promoting independence and resilience.
- Develop and maintain effective relationships with colleagues, health partners, and voluntary sector organisations to ensure integrated service delivery.
- Conduct assessments and reviews within statutory timescales, using person-centred and outcome-focused approaches.
- Address safeguarding concerns in accordance with the Wales Safeguarding Procedures 2019 and lead investigations as required.
- Prepare and present detailed reports at meetings, ensuring accuracy and coherence.
- Facilitate the active participation of adults and their families in care planning and decision-making processes.
- Maintain accurate records and manage a diverse caseload, ensuring timely communication and completion of tasks.
- Promote anti-discriminatory practice and provide guidance to less experienced practitioners.
- Attend and contribute to team meetings, fostering a collaborative environment for service development.
