What you will do:
- Undertake assessments in line with the Social Services and Well-being Act (Wales) 2014, focusing on children, adults, and their families or carers.
- Identify outcomes and develop care and support plans, while ensuring these are regularly monitored and reviewed.
- Evaluate and balance risks, promoting co-production, independence, and choice.
- Identify and promote strengths within individuals' circumstances, contributing to their overall well-being.
- Manage and work within allocated financial resources to deliver effective social care services.
- Promote social inclusion of isolated individuals, utilizing community and family resources.
- Collaborate with other agencies for a multi-disciplinary approach when necessary.
- Maintain and update electronic records, including case recordings, assessments, and care plans, following departmental guidelines.
Requirements of the Social Worker:
- A degree or equivalent qualification in social work.
- Current registration with Social Work Wales.
- Significant frontline experience as a qualified Social Worker within UK Social Services.
