What you will do:

Hold and manage a complex and varied caseload with appropriate supervision and guidance, ensuring compliance with policies and legislation

Prepare comprehensive reports and statements for judicial and Court of Protection hearings

Undertake complex decision-making and risk management using evidence-based tools and critical analysis

Communicate effectively and confidently in challenging situations with clarity and empathy

Attend, represent, and lead multi-disciplinary forums as required, working collaboratively across agencies

Conduct direct work with service users, carers, and families in line with safeguarding policies and procedures

Act as a care navigator for individuals with eligible social care needs, collaborating with health and partner agencies to develop support plans

Monitor and review support plans, identifying service gaps and reporting unmet needs appropriately

Work jointly with Social Care Assessors and delegate tasks in consultation with team leaders
