What you will do:

  • Manage a mixed caseload and provide high-quality, effective, person-centred assessments and interventions in line with the Care Act.
  • Partner with adults, their families, and carers to help them lead fulfilling and independent lives.
  • Focus on preventative and enablement approaches, delaying the need for long-term care by leveraging individuals' strengths and support networks.
  • Understand and operate within financial and legal frameworks, including Human Rights, Equalities legislation, and the Mental Capacity Act.
  • Effectively manage time and caseload demands while using data and technology to enhance your role.
  • Prepare clear and concise reports for meetings, including court and strategy meetings, adhering to set standards and deadlines.
  • Engage with stakeholders positively, ensuring alignment with organisational values and objectives.
  • Participate in regular supervision sessions to reflect on casework and identify areas for learning and development.
  • Stay current with research findings and innovative practices to maintain high professional standards.
