We are on the lookout for a skilled Locum Social Worker to join our vibrant Hospital Discharge Team in Greenwich. If you're keen to dive into the fast-paced world of hospital discharges and have a knack for ensuring safe and efficient transitions for patients, this role is perfect for you! Working full-time over a 3-month period, you'll play an instrumental role in the Discharge to Assess (D2A) process, collaborating closely with the multidisciplinary team to provide exceptional care and support.

What You Will Do:

Support safe, timely hospital discharges in line with D2A pathways and local discharge policies.

Conduct Care Act assessments, Mental Capacity Act assessments, and Best Interest decisions as required.

Develop person-centred discharge plans and coordinate community support, reablement, or short-term placements.

Collaborate effectively with multidisciplinary team colleagues, patients, and families for efficient discharge planning.

Identify and manage safeguarding concerns following statutory duties.

Maintain accurate and timely case records while managing a fast-paced caseload.
Essential Requirements:

Qualified Social Worker, registered with Social Work England.

Experience in hospital discharge, reablement, or adult social care.

Strong understanding of the Care Act, Mental Capacity Act, D2A pathways, and safeguarding procedures.

Ability to work independently and effectively within a multidisciplinary environment.
Salary range
