We are on the lookout for a skilled Locum Social Worker to join our vibrant Hospital Discharge Team in Greenwich. If you're keen to dive into the fast-paced world of hospital discharges and have a knack for ensuring safe and efficient transitions for patients, this role is perfect for you! Working full-time over a 3-month period, you'll play an instrumental role in the Discharge to Assess (D2A) process, collaborating closely with the multidisciplinary team to provide exceptional care and support.

What You Will Do:

  • Support safe, timely hospital discharges in line with D2A pathways and local discharge policies.

  • Conduct Care Act assessments, Mental Capacity Act assessments, and Best Interest decisions as required.

  • Develop person-centred discharge plans and coordinate community support, reablement, or short-term placements.

  • Collaborate effectively with multidisciplinary team colleagues, patients, and families for efficient discharge planning.

  • Identify and manage safeguarding concerns following statutory duties.

  • Maintain accurate and timely case records while managing a fast-paced caseload.

Essential Requirements:

  • Qualified Social Worker, registered with Social Work England.

  • Experience in hospital discharge, reablement, or adult social care.

  • Strong understanding of the Care Act, Mental Capacity Act, D2A pathways, and safeguarding procedures.

  • Ability to work independently and effectively within a multidisciplinary environment.

Salary range

  • £32 per hour
