What You Will Do

  • Deliver statutory services to adults with eligible care and support needs, ensuring assessments are person-centred, strengths-based, and outcome-focused.

  • Facilitate safe, timely discharges from hospital into home or community settings, working closely with health colleagues, families, and providers.

  • Manage a complex caseload and ensure all interventions, decisions, and plans are recorded accurately and on time.

  • Undertake Care Act assessment
