What you will do:

Assessment and Discharge Planning: Complete social work assessments for adults in hospital and coordinate safe, timely discharges in line with legislation and policy.

Safeguarding and Risk Management: Identify and manage adult protection concerns, complete risk assessments, and contribute to protection planning while promoting independence.

Multi-Disciplinary Working: Work closely with NHS professionals including doctors, nurses, and therapists, attending ward rounds and discharge planning meetings.

Legal and Policy Frameworks: Apply relevant Scottish legislation, including the Social Work (Scotland) Act 1968 and Adults with Incapacity (Scotland) Act 2000, and contribute to Mental Health and guardianship processes where required.

Recording and Professional Practice: Maintain accurate and timely case records and engage fully in supervision, training, and continuous professional development.
Requirements:

Qualification: Degree in Social Work or equivalent recognised qualification.
