What You Will Do:

Manage a caseload of up to 14 families, providing tailored support within the Family Help Team.

Undertake comprehensive assessments to understand needs, strengths, and risks, building individualised support plans.

Collaborate with multidisciplinary professionals to coordinate interventions and ensure cohesive support for families.

Maintain accurate, timely, and quality case records in line with statutory expectations and best practice.

Help families access community resources, empowering them to achieve positive change and sustainable outcomes.
Essential Requirements:

2-3 years' experience working with children and families within a Community Wellbeing Team (CWDT).
