What You Will Do:

  • Manage a caseload of up to 14 families, providing tailored support within the Family Help Team.

  • Undertake comprehensive assessments to understand needs, strengths, and risks, building individualised support plans.

  • Collaborate with multidisciplinary professionals to coordinate interventions and ensure cohesive support for families.

  • Maintain accurate, timely, and quality case records in line with statutory expectations and best practice.

  • Help families access community resources, empowering them to achieve positive change and sustainable outcomes.

Essential Requirements:

  • 2-3 years' experience working with children and families within a Community Wellbeing Team (CWDT).

Salary range

  • £39.04 per hour
