What you will do:

• Carry out comprehensive Care Act Assessments to ensure the support provided is in line with statutory requirements.
• Conduct outcome-focused reviews to assess the ongoing suitability of care plans.
• Undertake Mental Capacity Assessments and make Best Interest Decisions when necessary.
• Play a key role in supporting safeguarding enquiries under Section 42.
• Complete Continuing Healthcare (CHC) checklists and actively participate in Decision Support Tool (DST) meetings.
• Take part in duty rota responsibilities, ensuring effective service delivery.
• Collaborate effectively within a multidisciplinary team (MDT) environment, working alongside internal services and external partner agencies.

Qualifications / Requirements / Person Specification:

• Professional Qualification: You must be a qualified Social Worker with registration with Social Work England.
• ASYE Completion: Successful completion of the ASYE programme is essential.
• Relevant Experience: Previous experience in Adult Social Care and Complex Care is required.
• Legislative Knowledge: A robust understanding of the Care Act, Mental Capacity Act, and safeguarding legislation is vital.
• Core Skills: Ability to work independently and manage a diverse caseload with confidence
