What you will do:

  • Support the Team Manager in delivering strategic objectives and contributing to the development of community mental health services.
  • Undertake comprehensive assessments for adults and young people transitioning into Adult Services using person-centred outcome planning approaches.
  • Develop, implement, and review care packages in partnership with service users, carers, and partner agencies.
  • Manage an adult caseload, ensuring care plans remain effective, responsive, and focused on achieving positive outcomes.
  • Maintain accurate case records and performance data using departmental information systems.
  • Participate in regular supervision, mandatory training, and continuous professional development.
