What you will do

• Assessment & Care Management: Provide comprehensive assessments and care planning for adults within the CMHT setting.
• Multidisciplinary Working: Collaborate with health professionals, families, and community organisations to deliver holistic support.
• Care Planning: Develop and implement person-centred care plans that promote independence and wellbeing.
• Crisis Intervention: Respond effectively to crisis situations, supporting individuals to manage risks and make informed decisions.
• Case Recording: Maintain accurate, up-to-date records in line with statutory and organisational requirements.

Qualifications / Requirements

• Qualified Social Worker with current Social Work Wales registration.
• Experience working within adult social care, ideally within a mental health setting.
• Strong knowledge of relevant legislation and safeguarding frameworks.
• Ability to manage a caseload independently and work within a multidisciplinary team.
