What you will do:

• Undertake person-centred assessments, develop care plans, and complete reviews for adult service users

• Work collaboratively with partner agencies and professionals to deliver a holistic approach to community care

• Provide advice, support, and guidance to families and carers, signposting to appropriate services

• Maintain accurate, timely, and comprehensive records and reports in line with departmental requirements

• Travel across Helensburgh and Lochside to visit service users and attend meetings as required
