What you will do:

  • Conduct Care Act 2014 assessments for adults with cognitive and/or physical disabilities.

  • Develop and review person-centred, strengths-based care and support plans to maximise independence.

  • Apply the Mental Capacity Act 2005, completing capacity assessments and best interest decisions.

  • Work collaboratively with advocates, families, carers, and multidisciplinary professionals.

  • Identify and respond to safeguarding concerns, completing risk assessments and protection plans.

  • Liaise with health professionals, occupational therapists, and voluntary sector partners.

  • Support transitions

Salary range
