What you will do

  • Assessment: Assess the needs of children with disabilities and their families to identify appropriate support and interventions.
  • Care Planning: Develop, implement, and review personalised care plans that promote safety, wellbeing, and positive outcomes.
  • Family Support: Work closely with children, young people, parents, and carers to provide guidance, support, and advocacy.
  • Multi-Agency Working: Collaborate with education, health, and other partner agencies to coordinate services and resources.
  • Safeguarding: Identify and respond to safeguarding concerns, ensuring children remain protected and supported.
  • Case Management: Manage a varied caseload while maintaining high standards of practice and statutory compliance.

Qualifications / Requirements
