What you will do:

  • Conduct assessments and create care plans alongside children, families, and multi-agency partners to ensure a holistic and tailored approach to support.

  • Work collaboratively with educational, health, and voluntary sectors to ensure comprehensive support is provided.
  • Provide expert guidance and support to families, empowering them to understand and access the services available.
  • Maintain accurate records and reports to ensure compliance with Swindon Borough Council policies and procedures.
