What You Will Do:

• Complete assessments and develop tailored care plans for children with disabilities
• Work collaboratively with multidisciplinary teams including health, education, and external agencies
• Advocate for children and families in meetings and decision-making forums
• Monitor and review care plans, adjusting interventions as needs change
• Provide guidance and support to families navigating services and resources
• Maintain accurate and compliant case records in line with statutory requirements

Requirements:

• Qualified Social Worker with Social Work England registration
• Experience working with children with disabilities or within children's services
• Strong assessment, communication, and advocacy skills
• Ability to manage complex caseloads effectively
